Case # 7
A 75-year-old woman was admitted to the emergency department with dyspnea. She appeared lethargic, bradypsychic, with slowed verbal output. Her vital signs revealed a blood pressure of 120/64 mmHg, oxygen saturation of 80%, heart rate of 61 beats per minute, and body temperature of 37 °C. Facial and eyelid as shown on picture (Panel A). Chest computed tomography angiography revealed a large (3 cm) circumferential pericardial effusion (Panel B). Laboratory tests showed a thyrotropin level of 182.1 mIU/L (0.4-4.2), free thyroxin of 0.04 pg/mL(0.96-1.76).
